- To be signed by the CMH Heli-Ski guest on front and back of the form. (If applicant is under 19 a parent or legal guardian must sign here.)
- To be witnessed by a non-family member.
- Do not put any marks or highlights on the form other than the required information.
- Post to CMH NORDIC, PO Box 5896, 102 40 Stockholm, Sweden no later than one week after booking.

Instructions
Our experienced guides and pilots will hold a theoretical and practical safety training upon arrival for helicopter skiing to teach our skiing guests to be able to handle situations that may arise in the wilderness. All skiing involves risks that can not be completely eliminated, even by helicopter-assisted skiing ski lift. To conduct helicopter skiing in the province of British Columbia, it is required that ski guests fill out and sign a witnessed CMH Heli-Skiing waiver. If the form is incorrectly or inadequately completed, we can not accept your booking. Therefore it is important to follow the instructions for completing the waiver form.
